A Much Need Contribution to the Field

The bad old days of medicine: doctors understand biochemistry, genetics and pharmacology but have at most a glancing awareness of nutrition. Even doctors who deal with inborn errors of metabolism or with conditions being treated with ketogenic diets have limited understanding of nutrition - they just send patients to see a dietician or nutritionist.
The cutting edge now, and the doctor of the future will have an in-depth understanding of nutrition, be familiar with current research, be able to assess any patient's nutritional status and respond appropriately. That doesn't mean dieticians and nutritionists have no place, it simply means doctors have to step up to the plate and learn this important area of medicine more throughly. This well written book is an excellent place to start.
While specific to inborn errors of metabolism it does not limit itself to pediatrics. It covers adults with these syndromes who have survived into adulthood, and persons with imperfect penetrance (partial enzymatic activity) that usually are not even diagnosed until adulthood. That puzzling case you can't figure out could be explained thusly. MDs need to start assessing amino acid profiles, fatty acid profiles, carnitine levels and urinary organic acids on challenging cases.
